Part-Time Clinic Nurse (Bilingual English/Polish or Spanish)
Hours: 15 hours per week
Hourly Rate: $27 per hour
Location: Old Irving Park Community Clinic, 5425 W. Addison Street, Chicago, IL 60641
Position Summary
Under the supervision of the Clinic Manager, the Part-Time Clinic Nurse provides direct patient care, patient education, and care coordination in a community health clinic setting. The nurse plays a critical role in supporting clinicians, ensuring continuity of care, and delivering high-quality, compassionate services to uninsured and underinsured patients.
Key Responsibilities
· Provide direct nursing care to patients in accordance with clinic policies and professional standards
· Conduct patient assessments and document clinical findings accurately
· Review and communicate results from diagnostic testing
· Collaborate with clinicians to develop and implement patient care plans and case management strategies
· Provide patient education related to diagnoses, medications, and treatment plans
· Triage patient concerns, including phone calls and in-clinic needs
· Support care coordination and follow-up to ensure continuity of care
· Maintain accurate and timely medical records
Qualifications
· Current, unencumbered Illinois Registered Nurse (RN) license required
· BSN preferred
· One to two years of nursing experience preferred
· Bilingual English/Spanish or English/Polish proficiency required
· Strong written and verbal communication skills
· Ability to work independently and take initiative in a fast-paced clinic environment
· Commitment to serving diverse, underserved populations
