To provide a safe and healthy environment for staff, visitors and patients, Old Irving Park Community Clinic expects visitors, patients and accompanying family members to refrain from unacceptable behaviors that are disruptive or pose a threat to the rights or safety of other patients and staff. Our Patient Code of Conduct helps us to meet this goal.

AS A PATIENT VISITING OUR PRACTICE, PLEASE CONSIDER THE FOLLOWING:

·       Patients and visitors need to remain in “patient designated areas” (assigned treatment rooms or waiting area).

·       Patients, visitors, and staff will address each other in a respectful manner.

·       All patients and visitors will respect patient privacy and avoid disrupting other patients’ care or experiences.

·       Please communicate all issues that you wish to discuss with the doctor at the time your appointment is scheduled, so that an appropriate amount of time can be allotted. If you do not do this in advance, another visit may be necessary so that the doctor can give all patients the time and quality of care they deserve.

·       Our practice follows a zero-tolerance policy for aggressive behavior directed by patients against our staff.

·       Please be courteous with the use of your cell phone and other electronic devices. When interacting with any of our staff, please put your devices away. Set the ringer to vibrate before storing away.

·       Adults are expected to supervise their children. 

·       This is a smoke free establishment. Tobacco use is forbidden in the clinic.

THE FOLLOWING ARE PROHIBITED

·Weapons, firearms, illegal or dangerous items, alcohol, marijuana, and illicit drug use, as well as possession of related paraphernalia.

·Intimidating or harassing staff or other patients. This includes calling, texting or emailing staff to their personal numbers and accounts.

·Making threats of violence through phone calls, letters, voicemail, email or other forms of written, verbal or electronic communication

·Physically assaulting or threatening to inflict bodily harm.

·Making verbal threats to harm another individual or destruction of property.

·Damaging business equipment or property.

·Making menacing or derogatory gestures, together with sexual or vulgar words or actions.

·Making racial or cultural slurs or other derogatory remarks.

A safe and respectful environment is central to promoting a healing environment, therefore, if the above stated expectations are not followed:

Patients: If you choose not to comply with the above expectations, we may discharge you from the clinic or your appointment. For subsequent appointments or admissions, continuity of care would be determined by management and/or the clinic’s Board.

Visitors: If you fail to comply with the above expectations, you may be asked to leave, and may be restricted from future visitation privileges.

If you are subjected to any of these behaviors or witness inappropriate behavior, please report to any staff member. Violators are subject to removal from the facility and/or discharge from the practice.
